| Consist of Generic Drugs that can be purchased for $10.00 or less. |

| Consist of Brand Name and Select Generic Drugs that can be purchased for $40.00 or less. |

| Tier 4 |
| Consist of Brand Name and Select Generic Drugs that can be purchased for a special discounted price. |
